When Should I See A Doctor For Back Pain
When your back first starts to hurt, try taking an over-the-counter pain reliever and applying ice in the first 48 hours. You may apply heat after 48 hours. You may need to take it easy for a while, but its best to stay as active as tolerated, and to avoid absolute bed rest, said Dr. Guo.
If your back pain lasts more than two weeks and keeps you from participating in normal, daily activities, see your family doctor. If your pain is severe, you should see a doctor sooner. You should seek urgent medical care if you have:
- Fever associated with back pain
- Back pain after trauma
- Loss of bladder or bowel control
- Loss of strength in your arms and legs
- Unexplained weight loss associated with back pain
Also, always be more cautious if you have special risk factors for cancer, infection, or fractures that may affect the spine.
Drugs Are Not Part Of The Latest Recommendations For Treating Mechanical Back Pain
What the researchers found was surprising: for acute and subacute low back pain, the best and safest treatments are not medicines. The ACP made the following strong recommendation:
Most patients with acute or subacute low back pain improve over time regardless of treatment and can avoid potentially harmful and costly treatments and tests. First-line therapy should include nondrug therapy, such as superficial heat, massage, acupuncture, or spinal manipulation. When nondrug therapy fails, consider NSAIDs or skeletal muscle relaxants.
Because most mechanical back pain improves no matter what, we dont want to prescribe treatment that can cause harm. Because some medications carry significant risks, we really shouldnt be recommending these right off the bat. Rather, we should be providing guidance on heating pad or hot water bottle use, and recommendations or referrals to acupuncturists, massage therapists, and chiropractors. These therapies were somewhat effective, and are very unlikely to cause harm.

When Is Not Helpful
Back pain is one of the most common reasons why people visit a health care provider. The good news is that the pain often goes away on its own, and people usually recover in a week or two. Many people want to stay in bed when their back hurts. For many years, getting bed rest was the normal advice. But current studies recommend no bed rest at all and stress that staying in bed longer than 48 hours not only wont help but it may, in fact, actually delay your recovery. Heres why:
Staying in bed wont help you get better faster.If youre in terrible pain, lying down for a day to help ease the distress may seem like a good idea, but moderating your activities and staying active in a limited way is a more effective way to control your symptoms. Research suggests that if you can find comfortable positions and keep moving, you may not need bed rest at all.Research shows that:
- Lying down longer than a day or two day isnt helpful for relieving back pain.
- People can recover more quickly without any bed rest.
- The sooner you start moving, even a little bit, or return to activities such as walking, the faster you are likely to improve.
How To Get That Back Pain To Finally Go Away
So what does this mean for the treatment of low back pain? Our philosophy is that complex low back and pelvic floor patients need at least an hour of hands-on physical therapy with a pelvic PT who can address both the internal and external aspects of the issue.
However, it can be difficult to find this kind of hands-on treatment philosophy in todays medical world. Some patients do well when working with both a skilled orthopedic and pelvic floor physical therapist concurrently, treating both components of the problem at once. A massage therapist can also be a valuable tool in conjunction with a traditional, internal-only pelvic floor physical therapist. The massage can focus on the glutes, low back, and hamstrings while the pelvic PT resolves internal trigger points.

Fortunately, most people with back pain dont need surgery. We usually take a conservative approach first, using a wide variety of nonsurgical spine treatments, said Dr. Guo. For example, I might send you to physical therapy or chiropractic therapy. I might recommend medications such as anti-inflammatories, muscle relaxants, or some nerve-pain medications. We offer injection therapy, including epidural injections, joint injections, and nerve blocks and ablation if your back pain fails to improve with the conservative treatments. What You Can Do About Muscle Pain From Statins
Keep track of any new symptoms that develop when you begin taking a statin, and report them to your doctor. Some symptoms may go away as you continue to take the medication. If youre taking the statin preventively, your doctor may try to determine if your muscle pain symptoms are actually due to the statin by suggesting a brief holiday from the drug to see if they disappear when youre not taking it. However, dont stop taking a statin without telling your doctor. Although there are no proven remedies for statin-related muscle pain, the following may help.
Exercise. There is some evidence that people who have exercised regularly before taking statins are less likely to experience muscle pain and cramping. Although gentle stretching may relieve muscle cramps, beginning a new vigorous exercise regimen while taking a statin may increase the risk of muscle pain.
Ramp up your lifestyle changes. Commit to an exercise routine, lose weight if you need to, and adopt a heart- healthy eating plan such as the Mediterranean diet. Doing so may enable you to reduce your statin dosage, or possibly even lower your cholesterol enough to enable you to stop the statin. Even if a healthier lifestyle does not lower your lipid levels, it may still lower your long-term risk of heart disease and stroke.
Have a thyroid blood test. If you have a low thyroid hormone level, taking replacement thyroid hormone pills may alleviate muscle pain and can also improve your lipid profile.
How Are Back And Neck Pain Diagnosed
Your healthcare provider will ask about your health history and do a physical exam. They may also do X-rays of the affected areas, as well as an MRI. This allows a more complete view. The MRI also makes pictures of soft tissues such as ligaments, tendons, and blood vessels. The MRI can help spot infection, tumor, inflammation, or pressure on your nerve. Analyzing A Range Of Treatments For Low Back Pain
The American College of Physicians , the second-largest physician group in the U.S., recently updated guidelines for the management of low back pain. Its physician researchers combed through hundreds of published studies of non-interventional treatments of back pain, and analyzed the data. Treatments included medicines such as acetaminophen, nonsteroidal anti-inflammatory drugs such as ibuprofen and naproxen), opioids , muscle relaxants, benzodiazepines , antidepressants , anti-seizure medications , and systemic corticosteroids . The analysis also included studies on non-drug treatments including acupuncture, mindfulness-based stress reduction, tai chi, yoga, motor control exercise , progressive relaxation, biofeedback, low-level laser therapy, behavior based therapies, or spinal manipulation for low back pain.
Thats a lot of therapies!
