Supine Hip Shifting With Internal Rotation

What it does: This drill teaches you to simultaneously posterior tilt the pelvis while actively shifting the head of the femur to the back of the socket, both of which should decrease the sensitivity and the pinching that come with impingement, Kechijian says.

How to do it: Lie on your back with your knees bent at 90 degrees and your feet flat against a wall. Walk your feet out to each side as far as you can comfortably go , but keep your thighs parallel and roughly six inches apart . Tilt your pelvis back by lifting your tailbone slightly, while keeping your lower back flat on the ground. Hold this position, and gently tip your pelvis from side to side .

The knees should move up and down here, not forward and backward, while the lower back remains as stationary as possible on the ground, Kechijian says. The idea is to learn to dissociate the hips from the back.

Perform one to two sets of 30 to 40 shifts per side.

Building off of the concept of posterior chain it is important to consider the anterior chain as well.

Deficits in strength and flexibility can lead to several issues and lower back pain is most commonly coupled with weak and tight hip flexors. The hip flexors are the iliacus and psoas muscles. They are primarily responsible for lifting the femur into flexion .

When the pelvis is tilted forward through sitting, poor exercise technique, etc. there is a higher likelihood that the hip flexors will be in a shortened position as they attach to the lower spine and inside of the pelvis.

Over time this can cause weakness and tightness and encourage lower back pain. Fixing this issue at home takes time, but can be done through a hip flexor stretch.

Kneeling on the ground with the front foot put far out in front of you lunge forward while keeping your trunk over your hips and head up. The stretch will occur in the front of the thigh and hip toward the medial part of the leg that you are kneeling on. Hold this stretch for 30 seconds breathing in and out then switch to the other leg.

To do this exercise, find a prop to put your heels up. Lay your back on the floor and put heels on the bench. We are going to take advantage of a phenomenon known as Reciprocal Inhibition which sums up to when one muscle is flexed, the muscle on the opposite side tends to relax.

To begin hover your back off the floor and squeeze your glutes. Imagine that you are pulling your upper thighs in, to create inward tension. With glutes squeezed, maintain this squeeze, and do a crunch. At first it will feel weird but continue to reach forward and preform a crunch. This is a slow correctional exercise, so 2 seconds up, 2 second hold, 2 seconds down. Breath out on the way up, and inhale on the way down.

You know you are doing this right if you feel it in your glutes and your abdominals. Make sure to raise your glutes throughout the entirety of the exercise, and maintain hips of the ground.

Improve Psoas Strength When The Hip Is Flexed

The psoas muscles are two thick bands of tissue that connect the lower spine to the tops of your thigh bones. The psoas muscles are part of a muscle group known as the hip-flexors. Their primary function is to lift the thighs toward the torso allowing us to run, walk uphill, and climb stairs.

In movements requiring hip flexion , strong psoas muscles help stabilize your spine.

If your psoas is not working well when you try and hip hinge, your back will round, and this is what we want to avoid.

So, how do you activate and strengthen the psoas?

I recommend that you perform your favorite psoas activation exercise from these videos prior to performing your deadlift.

Lets begin with the alternating leg lift. Lie on your back, hands under your rear. Keeping your legs straight, lift one about 12 inches up from the floor and put it down again. 12 to 15 repetitions on each side should do it, and you can use ankle weights for progressive strength training.

Next, well do hanging knee lifts. Grasp a chinning bar, bring your knees to chest level and back down again, 15 to 20 reps is a good rep range to shoot for.

Finally, you can do V sits, which will work your psoas isometrically and develop hip stability. From a seated position on the floor, straighten your arms in front of you and lift your knees up.
