Dosage For People With Kidney Problems

Why does my lower back hurt? What causes and treatments of lower back pain?

Before prescribing Xarelto for you, your doctor will check to see how well your kidneys are working. This allows your doctor to know whether youll need renal dosing of Xarelto. Renal dosing is a reduced dose of the drug thats typically determined by calculating your creatinine clearance.

Creatinine is a waste product thats produced by your body. Your kidneys filter creatinine out of your blood, and it exits your body through your urine. Measuring the amount of creatinine in your urine allows doctors to see how well your kidneys are working.

Usually, your doctor wont prescribe Xarelto for you to treat or prevent a DVT or PE if your creatinine clearance is lower than 15 mL/min.

However, Xarelto can be used to reduce the risk of heart attack and stroke if you have kidney problems, no matter how low your creatinine clearance is. In this case, it can also be used to reduce the risk of heart attack, stroke, and other complications from blood clots.

If you have A-fib and your creatinine clearance is 50 mL/min or less, youll need a dose reduction of the drug. In this case, the recommended dosage of Xarelto is 15 mg taken once every evening.

Q: Are There Any Serious Xarelto Side Effects

Unfortunately, Xarelto can have an unintentionally severe impact on blood proteins, causing the blood to become extremely thin and leaving the patient susceptible to internal or external bleeding. As Xarelto is designed to prevent clotting, there is a risk that excessive bleeding may continue until Xarelto has exited the patient’s system.

Bleeding can cause serious health issues or even become fatal. For example, if there is bleeding near a vital organ such as the brain, lungs, or kidneys, then the normal blood flow to that organ may be interrupted. This disruption in the delivery of oxygen and vital nutrients could cause the organ to fail or otherwise lose some of its function.

The FDA advises that Xarelto patients tell their health care professionals if they have any signs of serious bleeding including:

  • Prolonged bleeding from wounds or gums
  • Unusual bruising
  • Abnormal menstrual flow – prolonged/heavy

Dosage For Preventing Dvt And Pe After Hip Or Knee Replacement

Xarelto is used as prophylaxis to prevent a DVT or PE following surgery done to replace your knee or hip.

For this use, youll start taking Xarelto between 6 and 10 hours after your surgery. Your healthcare professional will check to make sure that the bleeding from your surgery has stopped before you take your first dose of the drug.

Your dosage of Xarelto depends on whether youve had your hip or knee replaced. Typical dosages to prevent blood clots after these surgeries are as follows:

  • if youve had a hip replacement, the usual dosage is 10 mg taken once a day for 35 days
  • if youve had a knee replacement, the usual dosage is 10 mg taken once a day for 12 days

If youre prescribed the 2.5-milligram or 10-mg Xarelto tablets, youll take your doses with or without food. If youre prescribed the 15-mg or 20-mg Xarelto tablets, youll take each dose with food.

When you stop taking Xarelto, your risk for having a blood clot increases. If you have a condition that affects your arteries or heart, stopping Xarelto also increases your risk for heart attack and stroke. Examples of these conditions include peripheral artery disease and coronary artery disease.

Symptoms of a blood clot vary depending on the location of the clot and may include:

  • pain and swelling, which can affect a specific area of the body, such as the calf
  • trouble breathing

How Is Andexxa Used To Treat Xarelto Bleeding

If you develop severe bleeding, your doctor may treat it with an injection of a drug called Andexxa. This drug works to stop Xarelto from preventing blood clot formation in your body. Its used to treat severe, uncontrolled, or life threatening bleeding in people taking drugs such as Xarelto. Note: Xarelto has a boxed warning

Will I Need To Use This Drug Long Term

Why does low back pain appear? Causes and treatments ...

You might, but it depends on why youre taking Xarelto. The length of time Xarelto treatment is used is based on why youre taking the drug. Typical treatment lengths are as follows:

  • If youre taking Xarelto after having a knee replacement, youll take the drug for 12 days after your surgery.
  • If youre taking Xarelto after having a hip replacement, youll take the drug for 35 days after your surgery.
  • If youre taking Xarelto because you were admitted to a hospital for an acute illness, youll take the drug for 31 to 39 days.
  • If youre taking Xarelto to treat a deep vein thrombosis or pulmonary embolism , youll take the drug for 6 to 12 months.

However, when its prescribed to prevent blood clots or their complications, Xarelto is meant to be used as a long-term treatment. If you and your doctor determine that Xarelto is safe and effective for you, youll likely take it long term for these purposes.

If you have coronary artery disease or peripheral artery disease , youll take Xarelto with aspirin.

With CAD, you have narrowed arteries in your heart. With PAD, you have narrowed arteries in your legs. Both of these conditions increase your risk of having blood clots that can cause a heart attack or stroke.

Using a combination of Xarelto and aspirin is more effective for preventing heart attack and stroke in people with CAD or PAD than using aspirin alone.

The recommended dosage of aspirin for this use is 75 mg to 100 mg taken once a day.
