How Is Neuropathic Pain Treated
The goals of treatment are to:
- Treat the underlying disease .
Multimodal therapy is usually required to treat neuropathic pain.
Medicines commonly prescribed for neuropathic pain include anti-seizure drugs such as:
- Gabapentin .
Doctors also prescribe antidepressants such as
- Amitriptyline .
- Venlafaxine .
- Duloxetine .
Getting a prescription from your pain specialist for anti-seizure drugs or antidepressants does not mean you have seizures or are depressed. However, its true that chronic pain can be made worse by anxiety or depression.
Topical treatments like lidocaine or capsaicinpatches, creams or ointmentscan be used on the painful area. Opioid analgesics are less effective in treating neuropathic pain, and negative effects may prevent their long-term use.
The pain can also be treated with nerve blocks given by pain specialists, including injections of steroids, local anesthetics, or other medicines into the affected nerves.
Neuropathic pain that has not responded to the therapies mentioned above can be treated with spinal cord stimulation, peripheral nerve stimulation and brain stimulation.

Holistic And Osteopathic Specialists
In the event of chronic or severe back pain, many individuals tend to prefer to see osteopathic or holistic specialists. These types of doctors and professionals include osteopathic physicians and chiropractic specialists.
Osteopaths are put through the same training and licensing/certification processes that all medical doctors must obtain to practice medicine, but they tend to do additional studies of the musculoskeletal system. They then will prefer to treat pain problems medically, but with an additional emphasis on the potential lifestyle and environmental factors which may be contributing to the pain problem.
Chiropractors are trained in the proper alignment of the spine and may be able to provide some of the most immediate relief to back pain problems. This is done through periodic adjustments and treatments that attempt to maintain alignment and prevent further pain issues.

Coordinates Care Between Multiple Healthcare Professionals
Your pain management doctor acts as the lead in your pain management plan. As a post on ZocDoc explains:
The discipline is multidisciplinary, meaning that you dont just see a doctor but also will likely have the opportunity to interact with a physical therapist and a counselor. In this way, a comprehensive holistic plan for addressing your pain can be developed.
Once your pain management doctor has determined the types of therapies that could work for you, theyll coordinate your care across multiple doctors and healthcare professionals. While youll need to be a dedicated and responsive advocate for your care, you can put a lot of responsibility on your pain doctor and clinic for coordinating that care.

When To See A Doctor
There is not always an obvious cause of lower back pain, and it often gets better on its own. Resting, trying hot or cold therapy, taking OTC pain relievers, and gently stretching may help speed up recovery.
However, a person should see a doctor for lower back pain that is severe, does not seem to be getting better, or occurs alongside other concerning symptoms, such as tingling or numbness down the legs.
People with lower back pain should seek immediate medical attention if they also have any of the following symptoms:
- difficulty walking or moving the legs
- loss of bowel or bladder function
- loss of sensation in the legs
- very severe pain
A doctor can help a person identify potential causes of lower back pain and recommend appropriate treatment.
Best Doctors For Back Pain
Since you shouldn’t try to diagnose your own back pain, make your first call to a professional who can assess your problem, such as a primary care physician or a chiropractor.
Both can serve as an entry point for back pain.
Generally speaking, your primary care provider will only offer pain medications or injections designed to reduce pain, but not necessarily correct the issue.
Chiropractors on the other hand, have much more training when it comes to addressing back pain.
A chiropractors training contains over 150 more hours of education in biomechanics, while medical students learn more about pharmaceuticals.
Chiropractors use posture exercises and hands-on spinal manipulation to relieve back pain, improve function, and help the body heal itself.
According to Harvard Health, the most positive research on chiropractic therapy has focused on spinal manipulation for low back pain.
Here’s a few quotes including the scientific journal reference discussing chiropractic care for back pain:
âFor patients with chronic low back pain, clinicians and patients should initially selectnon-pharmacologic treatment with exercise, multidisciplinary rehabilitation, acupuncture, mindfulness-based stress reduction , tai chi, yoga, motor control exercise, progressive relaxation, electromyography biofeedback, low-level laser therapy, operant therapy, cognitive behavioral therapy, or spinal manipulation .â

Why Go To An Orthopedic Doctor For Back Pain
Most us will experience back pain at some point in life. While most back pains resolve on their own or with conservative treatments, some need serious medical attention. Back pain may be a sign of a serious condition, and it will progressively get worse if left untreated.
If your back pain is not getting better and is limiting the activities you can engage in, see an orthopedic doctor. Why go to an orthopedic doctor for back pain? Why cant your primary care physician just treat it? Orthopedic doctors have specialized knowledge, training, and experience in diagnosing, treating, and preventing musculoskeletal pain. While a primary care physician can treat a plethora of health issues, an orthopedic doctor can do it better.

A Pelvic Doctor Understands All Possible Complexities
Going to your primary care provider for CPP is like visiting your local dentist to get a root canal. You believe theyre qualified to help, but in reality, they lack the necessary specialized training. To have a root canal, youd need an endodontist and not a regular dentist.
In the same way, if you experience pelvic pain for more than six months straight, youll want to see a pelvic specialist. A pelvic doctor understands exactly how to deal with any problems related to pelvic pain, including musculoskeletal pain, endometriosis, vulvar pain disorders, painful bladder syndrome, and peripheral neuralgias, among others.
What Back Pain Treatment Is Available
Treatment of the back pain depends on if the problem is acute or if the problem is chronic .
Simple treatments like hot or cold packs, physical activity and exercise and physical therapy.
Pain medications like anti-inflammatories and anti-spasm medications to provide you pain relief.
If pain is long lasting , debilitating and has not been responding to treatments an assessment should be made by a pain management specialist, who can assess and treat most types of chronic pain.
Should I Visit A Pain Management Doctor
When should you visit a pain doctor?
- If pain is ruling your life or limiting the amount of activities you can engage in
- Youve tried at-home or natural therapies to relieve your pain and havent found relief
- If your primary care doctor hasnt been able to diagnose the cause of your pain
- Your pain has lasted for three months or more
- You have any severe or debilitating types of pain
As EverydayHealth explains:
As with any ailment, the first stop for patients looking for pain treatment should be their primary care physician. However, if you cant find a satisfactory pain management program within an appropriate length of time or if your pain is getting worse, referral to a pain specialist may be the next step.

How Is A Pelvic Pain Doctor Different From Other Specialists
If youve experienced pelvic pain before, you understand how debilitating it can be. Its especially common in women, but can also occur in men. Pelvic pain can be acute or chronic . Learning how a pelvic pain doctor is different that other specialists may help you find the care you are looking for.
While your gynecologist or personal doctor can treat acute pelvic pain with medication, chronic pelvic pain is better dealt with by a pelvic pain specialist. Unfortunately, theres a lot of misinformation out there about pelvic specialists, which can lead to a series of misdiagnoses and worsening pain.
If youre dealing with pelvic pain, or know someone who does, this article will explore pelvic pain and how a pelvic doctor differs from your local specialist.
Family And General Practitioners
When your neck or back pain first starts, your family doctor, general practitioner , or primary care physician is probably your best bet. They may prescribe some painkillers, give you a few exercises to do, and possibly send you to a physical therapist.
If your doctor deems your problem a serious one, they will likely order diagnostic tests and/or refer you on to a specialist such as a rheumatologist or a neurologist. But family doctors can be slow to include new back treatments as they come out.
According to a 2006 study published in Spine Journal. Other than taking your medical history and giving you a physical exam, which are two staples of spine diagnosis, the study found that even as new recommendations were published by leading medical organizations, physicians did not adopt them into their practices.
Because of this, taking a pro-active approach when shopping for spine care may be helpful to you. One way to do this is to research possible diagnostic and treatment options before seeking the doctor. Asking pointed questions while you’re at your appointment is another way.
Need Help Finding Doctor Who Specializes In Paraplegic Pain Issue
Hi I am writing to you to beg for some help. I have met someone at the college I am attending who was there as a motivational speaker. He is in a wheelchair due to a gunshot wound 19 years ago, he is now 45 years old. He and I really connected on a spiritual level and he helped me through a terrible time in my life. I have since come to realize that he is on the verge of taking his own life due to these pains that he gets, sometimes in excess of 15 hours a day, screaming, yelling, crying, begging god to make it stop. I am terrified that he will commit suicide and therefore I have been trying to find a specialist that would maybe listen to him and understand what he is experiencing. So far from what I can tell his doctors simply want to prescribe pain meds that do little if anything at all for him. Please help me to find someone who can possibly direct me in the right direction towards finding a specialist. Insurance is not an issue fortunately he has very good insurance its just finding someone to go to. He lives in Wilmington NC and though it would be nice to find a local doctor or one in state at least, travel is not a problem. Thank you for taking the time to read and respond to my message.
