OVER-THE-COUNTER PAIN RELIEVERS

Over-the-counter means you can buy them without a prescription.

Most health care providers recommend acetaminophen first because it has fewer side effects than other drugs. Do not take more than 3 grams on any one day, or over 24 hours. Overdosing on acetaminophen can cause severe damage to your liver. If you already have liver disease, ask your doctor if acetaminophen is OK for you to take.

If your pain continues, your provider may suggest nonsteroidal anti-inflammatory drugs . You can buy some NSAIDs, such as ibuprofen and naproxen, without a prescription. NSAIDs help reduce the swelling around the swollen disk or arthritis in the back.

NSAIDs and acetaminophen in high doses, or if taken for a long time, can cause serious side effects. Side effects include stomach pain, ulcers or bleeding, and kidney or liver damage. If side effects occur, stop taking the drug right away and tell your provider.

If you are taking pain relievers for more than a week, tell your provider. You may need to be watched for side effects.

Treating Your Back Pain

No matter what you suspect to be the cause of your pain, the first step in treating it should be to see your doctor. Usually, most short-term back pain improves over time without medical treatment. A typical course of treatment for back pain, which may be accompanied by muscle spasm, could include the following:

  • Bed rest for a day or two, though a long period of bed rest is not recommended
  • Local application of either heat or cold, depending on the type of injury,
  • A massage of the area, especially for a muscle spasm, and/or
  • An OTC anti-inflammatory pain medication such as Advil or a prescription anti-inflammatory drug and muscle relaxant.

How Does This Medication Work What Will It Do For Me

This is a combination product containing 2 medications: methocarbamol and ibuprofen. It is used to relieve pain associated with muscle spasm such as strains and sprains, back pain, and tense neck muscles.

Methocarbamol belongs to a class of medications called muscle relaxants that reduce muscle spasms. Ibuprofen belongs to a class of medications called nonsteroidal anti-inflammatory drugs that reduce pain and inflammation.

This medication may be available under multiple brand names and/or in several different forms. Any specific brand name of this medication may not be available in all of the forms or approved for all of the conditions discussed here. As well, some forms of this medication may not be used for all of the conditions discussed here.

  • Nonsteroidal anti-inflammatory drugs . You will see more improvement of your symptoms after 1 week of taking NSAIDS than taking nothing. Motrin/Advil or Aleve , all available without a prescription, are all good options for you. Ibuprofen can be taken 400 600 mg at a time, 4 times a day, and naproxen can be taken 220 mg twice daily for improvement of back pain. In studies, NSAIDS provide more relief of back pain than Tylenol . Try it as a first line choice.

  • Acetaminophen at higher doses, 1000 mg taken 4 times a day, does help modestly with back pain. Tylenol is better tolerated than NSAIDS, causing less stomach discomfort. This is your second choice after NSAIDS for the treatment of acute back pain. The usual warning here is not to take it for longer than 10 days at those doses.

  • What about both together? Studies suggests that there is little, if any, benefit of adding NSAID therapy to high-dose acetaminophen.

  • What about herbal options? Herbal non-steroidals may be effective. Willow bark extract works as an anti-inflammatory and may help for back pain. The bark of white willow contains salicin, which is similar to aspirin. In one study, the relief provided by willow bark was equivalent to the prescription COX-2 inhibitor rofecoxib .
