Pain Medicines: What To Do If You Have Heart Problems Or Kidney Disease

When people have pain, they often take pain medicines called NSAIDs . These include:

  • Advil and Motrin . Ibuprofen is also in other over-the-counter drugs, such as cold medicines.
  • Aleve .
  • Celebrex .

NSAIDs help ease pain and inflammation. But if you have high blood pressure, heart failure, or kidney disease, you should not take an NSAID. And you should not take any drugs that have ibuprofen or another NSAID in them. Heres why:

NSAIDs are bad for your blood pressure.

NSAIDs can cause high blood pressure. And if you have high blood pressure, they can make it worse. This increases your chances of having a heart attack or a stroke.

NSAIDs can also keep some blood pressure drugs from working right. NSAIDs can interfere with:

  • Diuretics, or water pills, such as apo-Hydro .
  • Diuretics remove excess water from the blood vessels.
  • ACE inhibitors, such as Altace and Coversyl .
  • ACE inhibitors are drugs that relax the blood vessels.
  • ARBs such as Cozaar . ARBs are another group of drugs that relax the blood vessels.

NSAIDs are bad for your heart and kidneys.

Long-term use of NSAIDs can make your body hold onto fluid. This can worsen the symptoms of heart failure, such as shortness of breath, swollen ankles, and a rapid or irregular heartbeat. NSAIDs can also keep the kidneys from working well. This makes taking NSAIDs risky for people who already have kidney disease.

Best Bets For Back Pain

Judicious use of pain relievers can help you keep doing your usual daily activities, which aids recovery from back pain.

When your back is bothering you and you don’t want to take prescription drugs, over-the-counter solutions and physical therapies can help relieve symptoms.

At this moment, about 10% of men have a backache, and up to 90% will have a backache at some point in their lives. Most flare-ups of low back pain get better over time, often within a few weeks. Pain control is important because it allows you to stay active, which assists in your recovery. For over-the-counter pain relievers, you have two options: acetaminophen or nonsteroidal anti-inflammatory drugs .

But pain relievers are only one tool to help you recover from, and prevent, low back pain, according to Dr. Jeffrey N. Katz, professor of medicine and surgery at Brigham and Women’s Hospital and editor of the Harvard Special Health Report Low Back Pain: Healing Your Aching Back. Dr. Katz suggests you also include these steps in your back-care plan:

Soothe with cold or heat. In the first few days, when pain is most intense and may be accompanied by inflammation, apply cold compresses for 15-minute periods. After a few days, switch to warm compresses to relax the affected muscles and enhance blood flow to the area. This simple approach can reduce reliance on pain relievers.

Acetaminophen Safety: Be Cautious But Not Afraid

Cold, cough, and flu season is a good time to revisit the risks of acetaminophenthe pain and fever reliever in Tylenol and many other over-the-counter medications. Billions of doses of acetaminophen are consumed safely every year, but deaths still occur from accidental overdoses and thousands of people end up in the emergency room. More than 600 products contain acetaminophen, and inadvertently combining them can nudge you into the red zone.

“People don’t realize that these doses all add up, and before you know it you’ve exceeded the recommended dose of acetaminophen,” says Dr. Melisa Lai Becker, instructor in medicine at Harvard Medical School and a specialist in emergency medicine and toxicology at Harvard-affiliated Cambridge Health Alliance.

Children can use naproxen, too. The OTC products are only labeled for children 12 years and older, but some prescription formulations can be used in children as young as 2 years of age for:

You must check with your doctor to use any form of naproxen in children under 12 years of age.

Naproxen should be used cautiously and at the lowest possible dose in the elderly, especially those at risk for, or with a history of stomach or intestinal ulcers or bleeding.

  • lithium.
  • The oral hypoglycemic drug is used to treat diabetes.
  • The dose of methotrexate is high.
  • A blood pressure lowering medication.
  • Angiotensin-converting enzyme inhibitors are used to treat angiotensin deficiency.
  • A beta blocker is a device that blocks access to beta content.
  • The diuretic action of diuretics.
  • Naproxen And Naproxen Sodium Are Available In Many Forms

You can take naproxen orally by tablet or as a liquid suspension, in an extended-release formulation, surrounded by an enteric coating, combined with a proton pump inhibitor to help protect your stomach, with a decongestant to relieve sinus pressure, or even combined with an antimigraine drug like sumatriptan. You can get it with a prescription, or without.

Naproxen comes in two forms – either as the base naproxen or as the salt form naproxen sodium. Both the base and salt forms work the same way to relieve pain. There is one difference — naproxen base on a milligram-per-milligram basis is slightly stronger than naproxen sodium. Heres how it works out — 220, 275, or 550 mg of naproxen sodium equals 200, 250, and 500 mg of naproxen, respectively. However, you dont need to worry about any conversion as these slight differences are already taken into account when doses are determined.
