How Do Exercises Help With Sciatica

Sciatica pain relief in 5min

According to the BMJ, healthcare professionals should see exercise as the main component of noninvasive treatment.

The reason for this is that, unlike some other forms of injury, sciatica pain may improve with training rather than rest. Additionally, continuing the exercises after the pain goes away may help prevent it from returning.

Factors contributing to the easing of sciatica symptoms may include the following:

Stretches Stretches To Help Relieve Your Pain

Piriformis Stretch

30 second hold 2 to 3 times daily

The sciatic nerve goes right below the piriformis muscle, so this is a key stretch for anyone experiencing sciatica symptoms.

Lie on your back with your knees bent.

Cross the affected sides ankle across the opposite knee so you make a figure 4.

Grasp your hands behind the unaffected leg and gently draw the thigh closer to your chest, creating a stretch in the affected sides hip/buttock area. Dont worry if its stiff and sore at firstthe pear-shaped piriformis muscle can become quite tight when the sciatic nerve is inflamed, so ease into this stretch, breathe and try to hold the stretch for at least 30 seconds.

Do this 2 to 3 times daily.

Knee to Opposite Shoulder Stretch

30 second hold 2 to 3 times daily

Lying on your back with legs straight, slowly bend the affected sides knee and hug the knee to your chest.

Hold this stretch, then draw across your body towards the opposite side to deepen the stretch. Breathe.

Hold for 30 seconds, repeat 2 to 3 times.

Prone Prop

5 holds 1 or 2 times daily

In many cases, sciatica can arise due to a disc bulge or herniation leading to a portion of the disc protruding outwards towards the spinal canal. In a young spine and a person who typically injures themselves by bending, lifting and twisting , youll want to apply the McKenzie Method to help centralise symptoms and promote a return of that disc into its space.

One of the most important stretches to do is the prone prop.

Main Causes Of Sciatica:

  • Spinal Stenosis which can compress on the nerves.
  • Extra bone growth near the sciatic nerve.
  • Spondylolithesis or misalignment of one vertebrae over another.
  • Lumbar or pelvic muscle inflammation or spasm can impinge a lumbar or sacral nerve resulting in sciatic symptoms.
  • Malignancy, infection, and gynaecological conditions .

If the press up with hips off center failed to offer significant relief or centralize your symptoms, you should then try the lumbar side glide exercise.

This exercise is done standing about a foot from a wall with your painful side away from the wall. Lean your shoulder against the wall with your elbow tucked into your ribs, and slowly push your pelvis underneath your ribcage.

Perform 10 repetitions of the side glide, and watch for changes in your pain. If your pain continues or fails to centralize, try the next exercise: the lumbar flexion rotation stretch.

Symptoms Of Sciatic Nerve Pain

The primary symptom, as mentioned above, is pain that starts in your lower back and radiates down your legs. Some other symptoms could point to sciatic nerve pain, though:

  • Numbness, tingling, or pins and needles in your legs
  • Burning sensation in your lower extremities
  • Pain that worsens with coughing, moving, or sneezing

How Can You Treat Sciatica

Bottom line: sciatica pain is not fun. Youll want to put the bear to sleep. Thankfully, sciatica often resolves with the right exercises, stretches, and adjustments to body mechanics that can help to push that bear to retreat back into its cave, so to speakin medical terms, the disc returning to its space.

That said, as always, treatment depends on whats really going on in your body, which requires a proper individualised assessment by a physical therapist and/or MD. Many factors such as age, medical history, and mechanism of injury can also help guide treatment.

Core stabilisation, education on proper lifting and bending techniques and likely some posture tips will certainly be an important part of treatment. But if you are in desperate need of a stretch or two to reduce your symptoms, these stretches are a good place to start.

    Tips For Coping With Sciatica

Sciatica is one of the most common, yet misunderstood, types of pain. As many as 40% of people will get it during their life, and it becomes more frequent as you age. Sciatica tends to get lumped in with regular back pain, but it is different. The pain originates with the sciatic nerves and often goes away by itself within a few hours or days. However, some attacks can come and go for several weeks or even months. Fortunately, there’s a lot you can do to prevent sciatica, as well as relieve the pain.

    Potential Risks & Drawbacks

Creams, regardless of type, can provide immediate inflammatory relief once applied because of their local application. All you have to do is rub the cream to the affected place, preferably, near the sciatic nerve roots and wait for ultimate relief to hit in.

Note: When it comes to risks of using sciatica creams, some of them may cause irritation, skin redness, and rash.

That can usually result because of an allergic reaction that hit in when you werent aware that youre allergic to some ingredients.

Another issue that some people report having is an upset stomach and other stomach-related problems due to sciatica topical medication.

Lastly, creams may not be as strong as oral medication and other treatments like tens of machines, which we also reviewed, so they provide only relief to minor symptoms.

    How Is Sciatica Diagnosed

The diagnosis of sciatica is based on your symptoms – in particular, pain that radiates or spreads down the leg. One common finding is that leg pain is usually worse than back pain.

Your doctor will also do a neurologic exam and some specific tests. One of the tests is the straight leg raise test, a specific test for sciatic nerve pain. The test checks for pain when your doctor raises your straight leg. If you have increased pain, the test is positive.

Imaging tests are recommended when a person has severe or worsening neurologic symptoms or when other serious conditions are possible.

    Muscle Release For Long

The above methods may only offer temporary relief from sciatica caused by muscle tightness and postural issues. To make sure the pain goes away, releasing both your tight piriformis AND tight hip flexor muscles may be the key for long-term sciatica relief.

You might be thinking But the hip flexors are on the front side of the body. How does that impact the sciatic nerve and cause sciatica pain down the leg? That is a great question, and heres why.

When the hip flexor muscles become tight from too much sitting, overuse in sports or activities, from prior injuries, and more they tip the pelvis forward into an anterior pelvic tilt. This can compress the discs and joints in the lower back and potentially pinch on a nerve.

Additionally, this anteriorly tilted position of the pelvis lengthens the muscles on the back side of your pelvis , making them more difficult to engage properly. When these larger muscles are not activating well enough, the much smaller piriformis muscle tries to pick up the slack. As you may imagine, the piriformis is quickly overworked and tightens up to where it may compress upon the sciatic nerve.

In this scenario, releasing only the piriformis might help in the short-term, but there is the potential for it to tighten back up again if the hip flexor muscles are not addressed. Since they are on opposite sides of the pelvis, these piriformis muscles like to play a game of tug of war with the hip flexors.

    • find it hard to start peeing, cannot pee or cannot control when you pee and this is not normal for you
    • do not notice when you need to poo or cannot control when you poo and this is not normal for you

These could be symptoms of a serious back problem that needs to be treated in hospital as soon as possible.

    Origin: Hips And Buttocks

Weve already covered how tight hips lead to low back compensation, resulting in back pain. Tight hips can also cause sciatica due to tense muscles in the middle of the buttocks your piriformis muscles putting pressure on the sciatic nerve. This condition is known as piriformis syndrome. If piriformis syndrome is the cause of your sciatic symptoms, releasing tension in your hips will take pressure off the nerve to relieve your symptoms.

The seated hip-opening twist in this video is designed to stretch your piriformis. Try this exercise slowly and carefully, paying attention to your form as you rotate. The twist needs to initiate from your midback. Do not force it, as compensating from your low back could exacerbate any lumbar disc issues. Stop if your pain worsens.

If you feel your sciatic symptoms abate while practicing this exercise, do two to three rounds once or twice a day for a few days to see how you progress. You should also try the hip-opening reclined and seated figure-four exercises included in the full video at the top of this article.

Manual soft tissue techniques, such as massage and foam rolling, can also provide some symptom relief, but the only way to prevent sciatica is by proactively using corrective exercise to strengthen, mobilize and realign the areas of your body that were causing the issue in the first place.

    Stretching And Physical Therapy

One of the best ways to treat sciatica pain and to prevent sciatica in the future is through physical therapy and stretching. With physical therapy, we can target and strengthen the surrounding muscles in your back to take the pressure off your sciatic nerve.

Stretching exercises while youre in the midst of a sciatica episode can also be highly beneficial. Sciatica is caused when your sciatic nerve is pinched or compressed, so anything you can do to relieve the pressure is good.

To get you started, here are a few stretches that can help with sciatica pain:

    Nerve Blocks And Epidural Injections

If your sciatica still presents problems after trying conservative treatments, we can turn to nerve blocks and/or epidural injections. These injections contain both anti-inflammatory and analgesic components and are extremely effective in providing relief from stubborn sciatica pain.

Bonus Tip: Get More Restorative Sleep

The quantity and quality of your sleep can directly affect pain and inflammation in your body. When you sleep, several inflammatory substances peak in your bloodstream and stay active throughout the night. When there is poor sleep quality, these inflammatory agents continue to circulate in the body through the dayâresulting in pain and fatigue.8

A full night of deep, restorative sleep can help decrease inflammatory pain and make you feel refreshed and rejuvenated.

If your sciatica pain gets worse and/or severely disrupts your daily life, check with your doctor about additional pain relief options, which may include medications, manual manipulation, epidural steroid injections, and more.

    Improved Soft Tissue Health

Regular mobilization exercises for sciatica may help heal the soft tissues in the disks and keep them healthy. Research suggests that the spinal disks may have a more healthy exchange of nutrients and liquid during exercise. As a result, prolonged disuse and reduced physical activity may be harmful to the disks.

    • Inflammation causing foods, such as sugars, refined carbohydrates, trans fats, and hydrogenated oils
    • Smoking, which can lower the production of anti-inflammatory molecules and induce inflammation

It is advised to check with your doctor before making extensive dietary changes and to ensure that your diet does not interfere or interact with any current medications.
