Piriformis Stretch

30 second hold 2 to 3 times daily

The sciatic nerve goes right below the piriformis muscle, so this is a key stretch for anyone experiencing sciatica symptoms.

Lie on your back with your knees bent.

Cross the affected sides ankle across the opposite knee so you make a figure 4.

Grasp your hands behind the unaffected leg and gently draw the thigh closer to your chest, creating a stretch in the affected sides hip/buttock area. Dont worry if its stiff and sore at firstthe pear-shaped piriformis muscle can become quite tight when the sciatic nerve is inflamed, so ease into this stretch, breathe and try to hold the stretch for at least 30 seconds.

Do this 2 to 3 times daily.

Knee to Opposite Shoulder Stretch

30 second hold 2 to 3 times daily

Lying on your back with legs straight, slowly bend the affected sides knee and hug the knee to your chest.

Hold this stretch, then draw across your body towards the opposite side to deepen the stretch. Breathe.

Hold for 30 seconds, repeat 2 to 3 times.

Prone Prop

5 holds 1 or 2 times daily

In many cases, sciatica can arise due to a disc bulge or herniation leading to a portion of the disc protruding outwards towards the spinal canal. In a young spine and a person who typically injures themselves by bending, lifting and twisting , youll want to apply the McKenzie Method to help centralise symptoms and promote a return of that disc into its space.

    The above methods may only offer temporary relief from sciatica caused by muscle tightness and postural issues. To make sure the pain goes away, releasing both your tight piriformis AND tight hip flexor muscles may be the key for long-term sciatica relief.

    You might be thinking But the hip flexors are on the front side of the body. How does that impact the sciatic nerve and cause sciatica pain down the leg? That is a great question, and heres why.

    When the hip flexor muscles become tight from too much sitting, overuse in sports or activities, from prior injuries, and more they tip the pelvis forward into an anterior pelvic tilt. This can compress the discs and joints in the lower back and potentially pinch on a nerve.

    Additionally, this anteriorly tilted position of the pelvis lengthens the muscles on the back side of your pelvis , making them more difficult to engage properly. When these larger muscles are not activating well enough, the much smaller piriformis muscle tries to pick up the slack. As you may imagine, the piriformis is quickly overworked and tightens up to where it may compress upon the sciatic nerve.

    In this scenario, releasing only the piriformis might help in the short-term, but there is the potential for it to tighten back up again if the hip flexor muscles are not addressed. Since they are on opposite sides of the pelvis, these piriformis muscles like to play a game of tug of war with the hip flexors.

    Weve already covered how tight hips lead to low back compensation, resulting in back pain. Tight hips can also cause sciatica due to tense muscles in the middle of the buttocks your piriformis muscles putting pressure on the sciatic nerve. This condition is known as piriformis syndrome. If piriformis syndrome is the cause of your sciatic symptoms, releasing tension in your hips will take pressure off the nerve to relieve your symptoms.

    The seated hip-opening twist in this video is designed to stretch your piriformis. Try this exercise slowly and carefully, paying attention to your form as you rotate. The twist needs to initiate from your midback. Do not force it, as compensating from your low back could exacerbate any lumbar disc issues. Stop if your pain worsens.

    If you feel your sciatic symptoms abate while practicing this exercise, do two to three rounds once or twice a day for a few days to see how you progress. You should also try the hip-opening reclined and seated figure-four exercises included in the full video at the top of this article.

    Manual soft tissue techniques, such as massage and foam rolling, can also provide some symptom relief, but the only way to prevent sciatica is by proactively using corrective exercise to strengthen, mobilize and realign the areas of your body that were causing the issue in the first place.
