Typically, doctors look inside the medicine cabinet to treat acute lower back pain. So what are the current options?

Multiple studies have looked at the effectiveness of treatment options for acute low back pain, including acetaminophen , nonsteroidal anti-inflammatory drugs like ibuprofen , narcotics like oxycodone, and muscle relaxers, or a combination thereof.

A British Medical Journal meta-analysis of 13 studies including 5,400 patients found that acetaminophen was ineffective at reducing back pain. Treatment with NSAIDs in the ER performed better ibuprofen, ketorolac and diclofenac all proved effective in reducing low back pain in one randomized-control study. However, the addition of a muscle relaxer to ibuprofen did not generate improvement in low back pain symptoms or mobility, according to a 2019 study in Annals of Emergency Medicine. And prior studies found no benefit to adding narcotics or the muscle relaxer diazepam to NSAIDs.

So NSAIDs perform the best in treating acute low back pain in adults.

Patients typically underdose ibuprofen aim for approximately 10mg/kg every eight hours with food as needed for pain. For an average adult, thats 600mg-800mg each dose.

What Are Your Options

If you try acetaminophen for a backache and it doesn’t work as well as you need it to, then stop taking it, says Dr. Katz. “Three to five days is all you need to find out if it will work.”

Also, you don’t necessarily have to choose between acetaminophen and NSAIDs you could try both at the same time. “They are often used in combination, because they don’t appear to have additive side effects, while many people find that they have additive benefits,” Dr. Katz says.

Finally, remember that pain relievers are just one tool available to men with back pain, whether occasional or chronic. Make sure to do all the other things mentioned above to recover from or prevent future flare-ups.

Ibuprofen Has Little Benefit In Treating Back Pain And May Cause Harm Study

Anti-inflammatory drugs are not much more effective than placebo and patients taking them 2.5 times more likely to suffer from stomach problems

Widely used anti-inflammatory drugs such as ibuprofen have little more benefit than a placebo when it comes to treating back pain, a comprehensive review has found.

Researchers analysed 35 peer-reviewed trials on the use of nonsteroidal anti-inflammatory drugs such as ibuprofen for back pain, reviewing data from 6,065 patients.

They found that none of the analgesics offered anything more than a mild relief for back-pain sufferers, and the effect was too small to be considered clinically important.

The comprehensive review, in the journal Annals of the Rheumatic Diseases, found while the drugs offered little to no benefit, patients taking them were 2.5 times more likely to suffer from gastrointestinal problems such a stomach ulcers and bleeding.

Lead author of the paper, Associate Professor Manuela Ferreira from the George Institute for Global Health in Australia, said back pain was the leading cause of disability worldwide and was commonly managed by prescribing anti-inflammatories.

But guidelines should be updated to reflect the drugs had little benefit, she said.

These drugs are effective for other conditions but for people with back pain, we believe there is a bigger role for other treatments, she said.

Thats one of the reasons we tend to use treatments, even when theyre not effective, he said.

The product called Advil Dual Action will be available nationwide later in 2020 and contains 250 mg of ibuprofen and 500 mg of acetaminophen, said GlaxoSmithKline, the drugs manufacturer, in a news release.
