    How Is Ankylosing Spondylitis Diagnosed

    There isnt a test that definitively diagnoses ankylosing spondylitis. After reviewing your symptoms and family history, your healthcare provider will perform a physical exam. Your provider may order one or more of these tests to help guide diagnosis:

    • Imaging scans:Magnetic resonance imaging scans can detect spine problems earlier than traditional X-rays. Still, your provider may order spine X-rays to check for arthritis or rule out other problems.
    • Blood tests: Blood tests can check for the presence of the HLA-B27 gene. About 8% of people of European descent have this gene, but only a quarter of them develop ankylosing spondylitis.

    Muscle spasms in the lower back can be very painful and debilitating. There is no definitive cause of back spasms, but they are often related to electrolyte imbalance, dehydration, or overuse of the muscles. Treatment for back spasms typically involves self-care measures such as gentle stretching, bed rest, and over-the-counter pain relievers. In more severe cases, physical therapy, chiropractic care, or massage may be recommended.

    If the exact cause of the muscle spasms can be identified and corrected, this is often the best solution. However, many cases of back spasms are idiopathic, meaning that the cause is unknown. In these cases, treatment focuses on relieving symptoms and preventing future episodes.

    Self-care measures for the relief of muscle spasms in the lower back include:

    • Applying a cold compress to the affected area for 15-20 minutes several times a day
    • Soaking in an Epsom salt bath for 15-20 minutes several times a week

    Ultrasound: An abdominal ultrasound is a simple and noninvasive test that requires no preparation or medication. Its usually all thats needed to locate gallstones. However, it doesnt visualize the common bile duct very well. If your healthcare provider suspects theres a gallstone hidden in there, they might need to use another type of imaging test to find it.

    MRCP:Magnetic resonance cholangiopancreatography is a type of MRI that specifically visualizes the bile ducts. Its non-invasive and creates very clear images of your biliary system, including the common bile duct. Your provider might use this test first to find a suspected gallstone there. But if theyre already pretty sure its there, they might skip it and go straight to an ERCP.

    ERCP: ERCP stands for endoscopic retrograde cholangiopancreatography. This test is a little more invasive, but its a useful one for finding gallstones because it can also be used to remove them from the ducts if they are stuck there. It uses a combination of X-rays and endoscopy, which means passing a tiny camera on the end of a long tube down your throat and into your upper GI tract.

    If you experience ongoing symptoms, it may be an indication that you have a deeper underlying anatomical problem in your spine.

    If spasms keep recurring or if there is ongoing inflammation or instability in your back, you should seek medical attention.

    How Are Back Spasms Treated

    Healthcare providers used to prescribe bed rest for those who deal with back spasms. Such inactivity is no longer recommended. Instead, follow your provider’s instructions about the following:

    • Ice/heat: Apply ice or heat to the location of your back spasms. Wrap the ice pack or heating pad in a towel or pillowcase and apply it to your skin for 20 to 30 minutes. Then reapply after 20 to 30 minutes.
    • Over-the-counter pain medications: Over-the-counter pain medications, including acetaminophen , naproxen or ibuprofen .
    • Muscle relaxants: Your provider may recommend taking a muscle relaxant like cyclobenzaprine, metaxalone or methocarbamol. If you do take them, take them at night and only for 72 hours or less.
    • Physical therapy: Your healthcare provider might prescribe physical therapy, where you can learn relaxation techniques and stretches for your back muscles.

    What Is Ankylosing Spondylitis

    Ankylosing spondylitis is a form of arthritis that causes chronic spine inflammation. Ankylosing spondylitis inflames the sacroiliac joints located between the base of the spine and pelvis. This inflammation, called sacroiliitis, is one of the first signs of AS. Inflammation often spreads to joints between the vertebrae, the bones that make up the spinal column. This condition is known as spondylitis.

    Some people with AS experience severe, persistent back and hip pain and stiffness. Others have milder symptoms that come and go. Over time, new bone formations may fuse vertebrae sections together, making the spine rigid. This condition is called ankylosis.

    Sciatica happens when the sciatic nerve in your spine becomes irritated and causes pain. Because this extra-long nerve extends down your legs, it can cause pain anywhere from the lower back to the toes. Pain can be mild or severe.

    Its typically a symptom of something else. A herniated disc, for example, can cause sciatic nerve compression or inflammation. Its also one of the most common reasons for this type of radiating nerve pain. Other conditions that may cause sciatica include:

    Iii Apply A Cold Pack

    For the first 48-72 hours, carefully apply a cold pack to the area to reduce any inflammation present. Protect the skin from any risk of ice burn with a thin towel before applying the pack. Apply for 20 minutes and use every 1-2 hours as needed.

    Caution:Not everyone benefits from a cold pack. If you find applying cold only numbs your pain while you are still stiff, then either reduce the length of time you apply it, or avoid cold treatment. Some people are more likely to feel even stiffer with cold treatment. The goal is to relax the muscle, not shock it.

    I recommend that you go with what works for you. Personally, I prefer heat instead, but everyone has their own preferences.

    How Is Ankylosing Spondylitis Managed Or Treated

    Ankylosing spondylitis is a lifelong condition. While theres no cure, treatments can prevent long-term complications, reduce joint damage and ease pain. Treatments include:

    • Exercise: Regular physical activity can slow or stop disease progression. Many people experience worse pain when theyre inactive. Movement seems to lessen pain. Your healthcare provider can recommend safe exercises.
    • Nonsteroidal anti-inflammatory drugs :NSAIDs, including ibuprofen and naproxen , ease pain and inflammation.
    • Disease-modifying anti-rheumatic drugs : Medications such as sulfasalazine reduce pain and joint swelling. The drugs also treat lesions caused by inflammatory bowel disease. Newer DMARDs called biologics help control inflammation by changing the immune system. Biologics include tumor necrosis factor and interleukin inhibitors .
    • Corticosteroids: Injectable corticosteroids temporarily ease joint pain and inflammation.
    • Surgery: A small number of people with ankylosing spondylitis may need surgery. Joint replacement surgery implants an artificial joint. Kyphoplasty corrects a curved spine.
