How To Relieve Hip, Lower Back and Buttock Pain: Piriformis Myofascial Release Technique

If you are experiencing an achy back, your hips may be to blame.

Tight hips can cause discomfort or stiffness in the lower back, groin, hip or thigh, which is worse with stairs, squats or other activities that require active hip flexion, says Sal Frangiamore, M.D., an orthopedic surgeon for the Summa Health Orthopedic Institute and team physician for the US Ski and Snowboard Association .

Dr. Lev Kalika, owner of New York Dynamic Neuromuscular Rehabilitation & Physical Therapy in New York City, adds: The low back is intimately connected to our hips. The lumbar spine sits on top of the pelvis, and the hips and pelvis cannot be separated functionally. Because of this, Dr. Kalika explains that hip mobility prevents over recruitment of back muscles.

You may not be able to prevent lower back pain, especially as you age and your back loses some strength and resilience. Fortunately, there are many ways you can get relief, no matter the cause of your back pain.

What Are Some Other Causes Of Lower Back Pain

The shape of your spine, and well as spinal diseases, are other culprits in lower back pain. Depending on a range of factors, your doctor may look for:

Abnormal spinal curvature. A normal spine resembles a gently curved letter S when seen from the side. Abnormal curves include:

  • Lordosis, in which the spine curves too far inward at the lower back
  • Kyphosis, in which the spine is abnormally rounded in the upper back
  • Scoliosis, in which the spine curves from side to side, often in a C shape

Normal and abnormal curves of the spineArthritis. There are more than 100 types of arthritis, many of which can cause lower back pain. The most common types include osteoarthritis , rheumatoid arthritis, and ankylosing spondylitis.

Cauda equina syndrome . Compression of the bundle of nerves that forms below the spinal cord in the lumbar spine. It is a rare but serious disorder that requires immediate medical attention and possibly emergency surgery. CES got its name from the fact that the fanned-out bundle of nerves resembles the base of a horses tail.

Discitis or osteomyelitis. Both infections of discs and bone can cause severe pain and require prompt medical attention.

Spinal tumors. When cells divide and multiply unchecked, the result is a tumor. Both benign and malignant tumors can cause lower back pain. They can either originate in the spine or metastasize there, meaning theyve spread from somewhere else in the body.

Core Stability Pelvic Tilt

Lack of core control and stability is a key player in lower back pain. When the abdominal muscles are not performing, the muscles of the lumbar spine have to work harder to stabilize the body in balance. An excellent way to improve core stability is to lie flat with the knees bent up. Tighten abdominal muscles and press the small of the back into the floor. Hold this contraction for 10 seconds and repeat 10-20 times. You can either have your arms resting by your sides or behind your head, depending on whats more comfortable. With this stretch, it is important remember:

  • Keep your feet flat on the floor
  • Keep your shoulders pressed to the floor, but relaxed
  • Do not hold your breath as you perform this exercise

While lower back pain is extremely common, it is not something you have to live with every day. Improving hip flexibility and core stability as well as lengthening the back muscles by stretching can help reduce the symptoms of lower back pain. However, if your back pain is chronic, the above stretches may help but additional treatment may be warranted.

Another reason you may be feeling pain in one of your hips is that when you have a pinched nerve in your back that affects one side of your lower body, you tend to favor that side. The pain may alter the way you move, which can place added stress on your already hard-working hips.

So, if your hip pain is more of a dull ache and youre also experiencing some of the more classic signs of sciatica, the two are likely related.

What Are Some Less Invasive Or Noninvasive Back Pain Treatments

Your doctor has a wide range of treatments that may help your lower back pain. In general, expect your doctor to take a stepped care approach. That means starting with simple, low-cost treatments and moving to more aggressive approaches later. Keep in mind that many treatments take time to reach their full effect.

Medications. When over-the-counter pills and topicals dont do enough to relieve back pain, your doctor may recommend a prescription drug. Examples include:

  • Antiseizure medication, such gabapentin or pregabalin, for nerve-related pain
  • Muscle relaxants, such as baclofen or carisoprodol
  • Prescription NSAIDs, such as celecoxib, diclofenac, or fenoprofen
  • Opioids, such as oxycodone or hydrocodone, on a short-term basis.

Physical therapy . PT for lower back pain involves passive and active therapies to help the patient build core muscle strength, improve spinal flexibility and range of motion, correct posture and more. Your physical therapy sessions may include:

Injections. An epidural steroid injection or a selective nerve block may provide short-term pain relief when lower back pain causes sciatica symptoms, such as leg pain.

Surgical interventions to correct spinal deformity include spinal fusion, which is a surgical procedure that uses bone graft to replace a disc. After undergoing surgery, your back will require physical therapy for three to six months. In addition to doctors and dentists, you can also see neurologists, physiatrists, and chiropractors. These specialists specialize in the brain and spinal cord disorders. They are often the best choice for patients seeking treatment for back pain.

Non-specific back pain is usually not diagnosed until the pain is long-term. There is no clear cause for non-specific back pain, but doctors should seek a diagnosis to treat the condition. It is important to visit a specialist for treatment in order to get the best treatment options. If the pain is chronic or has no specific cause, a doctor will perform diagnostic imaging to help identify the exact source of the pain. Further, they can perform MRIs to help diagnose underlying conditions.

While back pain is a common ailment, it can also be caused by something else. Many people experience sciatica or other pain in the leg, and a slipped disc could be the cause. Other types of back pain may be more acute, such as a herniated disc. For nonspecific back pain, a doctor may need to perform radiological imaging to diagnose the exact cause of the pain.

Try These Simple Exercises To Help Relieve Lumbar Spine Pain

Back pain affects 80 percent of us at some point in our lives. The good news is that for many people suffering from low back pain, exercises to stretch the hip flexor muscles can help.

The hip flexors are a group of muscles than run from your thighs all the way up to your low back. They include muscles with strange-sounding names such as the ilacus, the psoas, and the rectus femoris. As a group, these muscles let you bend at the waist and bring your knee to your chest.

The problem is that many of us spend our days sitting. When we do that, these muscles are always in a state of contraction. This causes them to shorten and become weaker.

One of the hip flexor muscles, the psoas is attached to your lumbar vertebraethe five bones that form the spine in your lower back. So you can see why tightness or weakness in this muscle could affect your back.

Stretching the hip flexor muscles can help prevent and treat some causes of lower back pain. If youre having low back pain and you think it might be a result of tight or weak hip flexors, spending a few minutes a day on some simple stretches could help.

Before you start any exercise program, of course, check with your doctor to make sure youre in good enough health to perform the exercises without hurting yourself and that there arent underlying conditions that need to be treated.

While you cant stop aging or change your genetic makeup, lifestyle changes can help manage and prevent lower back pain. Having a healthy lifestyle may make you less likely to suffer an accidental injury, too.

Improve your physical fitness. People who are not physically fit are more likely to develop lower back pain because strong core muscles help support the lower back.

Stay active. People who lead sedentary lives may be more likely to injure themselves when they do exert themselves. As far as back health is concerned, its better to do a smaller amount of physical activity most days of the week than to sit all week and over-exert yourself on the weekends.

Lose weight if necessary. The more you weigh, the more pressure you have on your lumbar vertebrae. Being overweight or obese can put stress on the back and lead to lower back pain.

Lift heavy objects the right way. Be sure to squat while lifting so that your hips and knees do much of the work. Keep the load close to your chest while you lift.

Your legs, not your lower back, should be the main driver when you’re lifting something heavy.Make your workspace as ergonomic as possible. If you sit at a desk, make sure your chair has plenty of low-back support and your hips are at a right angle to the floor.

Dont use tobacco products. In addition to all the other health problems tobacco use causes, it can restrict blood flow and oxygen to the discs, causing them to degenerate faster.
