    Abnormalities in x ray and magnetic resonance imaging and the occurrence of non-specific low back pain seem not to be strongly associated.w6 Abnormalities found when imaging people without back pain are just as prevalent as those found in patients with back pain. Van Tulder and Roland reported radiological abnormalities varying from 40% to 50% for degeneration and spondylosis in people without low back pain. They said that radiologists should include this epidemiological data when reporting the findings of a radiological investigation. Many people with low back pain show no abnormalities. In clinical guidelines these findings have led to the recommendation to be restrictive in referral for imaging in patients with non-specific low back pain. Only in cases with red flag conditions might imaging be indicated. Jarvik et al showed that computed tomography and magnetic resonance imaging are equally accurate for diagnosing lumbar disc herniation and stenosisboth conditions that can easily be separated from non-specific low back pain by the appearance of red flags. Magnetic resonance imaging is probably more accurate than other types of imaging for diagnosing infections and malignancies, but the prevalence of these specific pathologies is low.

    How Is Back Pain Diagnosed

    Most people who develop low back pain that comes on suddenly have nonspecific low back pain. If there are no other associated symptoms and the pain is not too bad, many people are confident to just ‘get on with it’ and treat it themselves – and indeed most get better quickly. However, if in doubt, see your doctor for a check-over and advice.

    A doctor will usually want to ask questions about your symptoms and to examine you. Basically, the symptoms are usually as described above, with no other worrying symptoms to suggest anything serious or another cause of back pain . The examination by a doctor will not detect anything to suggest a more serious cause of back pain. Therefore, a doctor can usually be confident from his or her assessment that you have nonspecific back pain.

    As a general guide, if any of the following occur then it may not be nonspecific low back pain, and there may be a more serious underlying cause. But note: the vast majority of people with low back pain do not have any of the following symptoms or features. They are included here for completeness and as an aid to what to look out for and to tell your doctor should they occur.

  • Symptoms that may indicate cauda equina syndrome. The main ones are, in addition to back pain:
  • Numbness around the back passage – the saddle area.
  • Bladder symptoms such as loss of bladder sensation loss of bladder control, incontinence, loss of sensation when passing urine.

    6. Spondylolisthesis: spondylitis occurs when a vertebra in the lower spine slides out of place, pinching the nerves that leave the spine.

    7. Traumatic injury: it is caused by sports, accidents, or falls that can injure the tendons, ligaments, or muscles.

    8. Spinal stenosis: It is a narrowing of the spinal column that puts pressure on the spinal cord and nerves. It can cause pain or numbness when walking and, over time, can lead to weakness and sensory loss in the legs.
