Acute low back pain usually gets better on its own, and no treatment has been shown to improve the outcome much beyond its natural course.3

However, other medicines for pain relief include anti-inflammatory medicines such as ibuprofen , aspirin and diclofenac . These may give a small amount of relief, but they are more likely to cause side effects than is paracetamol.8

Some people, including those with high blood pressure, asthma or stomach ulcers, or who take other medicines have a higher risk of side effects from NSAIDs. Before taking any NSAIDs, including those bought over the counter, ask your doctor or pharmacist about the benefits and potential risks.

There are non-medicine treatments available for back pain, but many have not been studied in detail and so it is not known how helpful they are.8 However, this does not necessarily mean they will not help you to manage the pain. The Tylenol label warns that taking acetaminophen can cause liver disease. Because of this, the label says, if you take more than 3 drinks every day you should talk to your healthcare provider about also taking acetaminophen.

The BMJ study mentioned above also found high-quality evidence that people who take paracetamol every day are almost 4 times as likely to have abnormal liver function test results than those who took a placebo pill. Just the same, the authors say that the clinical relevance of the abnormal liver tests are unclear.

Allergy to acetaminophen and/or other ingredients in the medication is possible. If you have a known allergy to any of the ingredients listed on the box, dont take this medication. If you’re unsure, talk to your healthcare provider. And if you do have an allergic reaction after taking acetaminophen, seek medical attention immediately.

Ibuprofen and Naproxen are also very popular over-the-counter pain relievers and fever reducers. They belong to a class of drugs called Non-steroidal Anti-inflammatory Drugs . Similar to Tylenol, these drugs are also used for many types of pain including headaches, muscle aches, sprains and strains , menstrual pain, and certain types of arthritis. They are also found in many cough and cold medications because they can reduce fever in both adults and children.

So which is better for back pain: Acetaminophen or an NSAID ?

As you can see from this article, NSAIDs such as ibuprofen or naproxen are more effective than acetaminophen for low back pain. This is in part due to the fact that NSAIDs can reduce inflammation in your lower back, whereas Tylenol does not.

However, if you have medical reasons that put you at risk of side effects with NSAIDs , you should try Tylenol instead. Tylenol is generally safe. Do not take Tylenol with alcohol and avoid it if you have liver disease.

To prevent side effects from NSAIDs such as ibuprofen or naproxen, you should take the lowest effective dose for the shortest duration necessary . You should also take ibuprofen or naproxen with food to minimize stomach upset.

How Tylenol Arthritis Works

Tylenol Arthritis contains two layersone that releases quickly for fast relief and one that releases slowly to provide lasting relief of up to eight hours. For these layers to work properly, you must swallow the caplets whole with water. There is a total of 650 mg of acetaminophen in each tablet. Regular Strength and Extra Strength Tylenol do not have two layers and contain less acetaminophen per caplet, 325 mg, and 500 mg respectively.

Acetaminophen is an analgesic, meaning it relieves pain. Acetaminophen is not an anti-inflammatory, so it does not ease swelling. For this reason, Tylenol Arthritis is best for osteoarthritis and not inflammatory types of arthritis such as rheumatoid, reactive, or gout.

Question: I suffer from osteoarthritis. My doctor says I should keep exercising even though my joints hurt. He says its okay for me to take a pain medication like Tylenol in order to remain active. But Ive read that Tylenol can damage my liver. What am I to do?

Answer: You are right to be concerned about medication safety. But your doctors also right that you need to be physically active. So how do you balance these seemingly competing objectives?

I posed your question to two specialists at Sunnybrook Health Sciences Centre: One is a surgeon who performs joint replacement operations and the other is an expert in drug safety.

Lets start with the surgeon. Dr. Jeffery Gollish, Medical Director of Sunnybrooks Holland Orthopaedic and Arthritic Centre, says he sees lots of people who feel the same way you do.

Many patients, he says, fear that exercise will lead to additional damage of their joints. The fact that it hurts when they move is taken as a sign that they are doing harm to themselves. And they worry medications that mask their pain will simply allow them to do even more harm.

The issues of exercise and medication are two areas where the public has misconceptions, says Dr. Gollish. They think both are harmful. In fact, both are beneficial.

Exercise, in general, is good and wont harm the joints. The only thing we tell people to stop doing is running and avoiding any impact activity.

Ibuprofen Or Naproxen Effectiveness For Back Pain

Ibuprofen or Naproxen belong to a class of drugs called NSAIDs that are first-line treatments for back pain. According to one study:

  • The evidence from the 65 trials included in this review suggests that NSAIDs are effective for short-term symptomatic relief in patients with acute and chronic low-back pain without sciatica

Thus NSAIDs such as naproxen and ibuprofen are modestly effective for low back pain.

The doses you can try are:

  • Ibuprofen 400mg to 600 mg four times daily or
  • Naproxen 220mg to 440mg twice daily

You should take the dose on a scheduled basis rather than as needed for about 1 to 2 weeks. Taking an NSAID on a schedule basis for about 1 to 2 weeks will really decrease the inflammation in your back. After 1-2 weeks, you can take the NSAID on an as needed basis.

You do not want to take an NSAID like ibuprofen or naproxen longer than 2 weeks without the supervision of your doctor because these medications have lots of side effects if you take them constantly for a long time. The safety of NSAIDs shall be discussed below.

Drugs And Medications To Manage Spinal Stenosis Symptoms

Over-the-counter Pain MedicationsOver-the-counter drugs are usually the first line of medication treatment for spinal stenosis. These medications may help alleviate pain and/or inflammation.

Medications that control pain are known as analgesics, and they are available in OTC and prescription forms. Acetaminophen , aspirin, ibuprofen , and naproxen are examples of OTC analgesics that your doctor may recommend for spinal stenosis.

While some analgesics only relieve pain , others reduce pain and inflammation. Analgesics that reduce both pain and inflammation are called non-steroidal anti-inflammatory drugs, or NSAIDs. Aspirin, ibuprofen, and naproxen are examples of NSAIDs.

Prescription Drugs for Spinal StenosisIf your spinal stenosis symptoms are not responding to OTC medication, ask your doctor if a prescription drug may provide better pain management.

Spinal stenosis in your low back can produce symptoms of numbness, tingling, and burning down your legs. A neuropathic agent may reduce nerve pain and help restore function.

Finding The Right Treatment

Most cases of lower back pain are comparable to the common cold.

Depending on each patients unique situation and condition, conservative treatments and over-the-counter pain relievers may not address the underlying problem. In these cases, stronger medications or perhaps surgery might be the answer. Our goal is to provide the right treatment for the right condition, for the right patient, at the right time.

Christine Lin, a researcher involved in the study, explained to Reuters that even though acetaminophen may not be likely to provide relief from lower back pain, there is evidence that it works to relieve pain from a range of other conditions such as headaches, some acute musculoskeletal conditions, tooth ache, and for pain straight after surgery, Other experts not involved in the study were hesitant to advise patients to retire acetaminophen from their back pain regimen, commenting that more robust and consistent evidence, including verification of the results in other populations, is needed.
