Managing Pain While Treating Your Cancer

Cancer pain can be traced to a variety of events, which sometimes makes it difficult to control. It may be the growing tumor thats causing the pain, but cancer also triggers a chemical release that can affect nerves and increase pain. In some cases, the cancer treatment itself damages the nerves and can leave painful scarring.

There are several approaches to treating back pain while you continue with your cancer treatment plan, but not every method will work well for you. Over-the-counter prescription medications may be able to control your discomfort, or if you need something stronger, your doctor may provide you with an opioid medication. However, dont discount the benefit of hands-on therapies like acupressure, massage, and chiropractic treatment that can target specific areas where nerves or muscles may be suffering.

Be sure to take your doctors advice about introducing a new therapy, but try to stay open-minded about different approaches to back pain relief. Pain is a very individual experience, and it can be difficult to predict what will ultimately help you most.

Kidney Pain Vs Back Pain: How To Differentiate

According to various data banks, the overall prevalence of low back pain was found to be 42% in India. Kidney problems too result in back pain which may be a little different from musculoskeletal back pain. In todays write up we will find out how to differentiate between the two.

Each person has got a pair of kidneys located posteriorly on the lower side of the abdomen. The kidney purifies blood for nutrients and discharges urine.

How to differentiate kidney pain from general backache:

  • Location As is the location of kidneys the pain will be felt on the posterior lower side of the abdomen usually on one side . It may rarely occur on both sides too.
  • Musculoskeletal back pain is usually felt around the lumbar region, it may pain while muscles are touched. However musculoskeletal back pain can be felt all through the back as well.

  • Radiation of pain Kidney pain may radiate to the inner thigh or lower abdomen.
  • Musculoskeletal back pain due to disc or nerve impingement may radiate to buttocks, back of the thigh, laterally to leg and ankles too.

  • Type of pain Mostly a constant steep pain or dull aching pain. The kidney stone pain may vary as the stone moves
  • The musculoskeletal back pain is mostly coming and going, peaking after high-intensity work and better with rest. Pain here too is dull aching.

  • The severity of pain Mostly a constant steep severs pain or mild aching pain .
  • The musculoskeletal back pain is not very severe.

  • Symptoms of both:
  • Advanced Or Recurrent Kidney Cancer Treatment

    • Medicine is often used for advanced kidney cancer that has spread to other parts of the body or where surgery cannot be done.
    • Immunotherapy uses the bodys defense system to stop or slow the growth of cancer cells
    • Monoclonal antibodies attack a specific part of cancer cells
    • Checkpoint inhibitors help the immune system recognize and attack cancer cells
    • Vaccines give an overall boost to the immune system
  • Anti-angiogenic therapies reduce the blood supply to a tumor to slow or stop its growth

Is Back Pain An Indication Of Kidney Cancer

A sudden or unusual ache in the middle or lower portion of the back might have a person looking up all kinds of possible medical ailments online.

Back pain is one of the most common reasons people call a doctor, but the odds are very good that the cause will not be kidney cancer, says Eric Kauffman, MD, a urologist with Roswell Park Comprehensive Cancer Center.

Kidney cancer is usually asymptomatic. We find it most often through scans that are attained for other reasons, Dr. Kauffman says. The most common symptom we see is blood in the urine. Its uncommon, but thats the most common symptom. Back pain in general is uncommon with kidney cancer because we dont have the same kind of pain receptors on the kidney like we do on our skin or muscle. You dont feel when things invade there.

How Are Back Pain And Kidney Cancer Related

Before the advent of CT scans, which use X-ray technology to detect a variety of diseases and conditions, a kidney cancer mass could grow large enough to push on muscles in the back or spread to the bones, causing pain. This is where the symptom got its reputation, though now its become an old medical textbook finding that we just dont see anymore, McGregor explains.

With todays screening advancements, doctors are often able to identify kidney cancer at an earlier stage, typically while conducting scans for other reasons, and well before symptoms such as back pain present.

A Mass Or Lump Around Your Abdomen

5 symptoms of kidney cancer everyone should be aware of

A mass or lump in the abdomen, side, or back can also be a sign of kidney cancer. You might feel a hard thickening or bulging bump under your skin.

But kidney lumps are hard to feel, especially in the early stages. Thats because the kidneys sit deep in the abdomen. You may not even see or feel the lump as the tumor grows.

If you do discover a lump, a doctor or other HCP will likely order diagnostic tests, such as an ultrasound or a CT scan. These tests may help determine the cause of the lump. Youll typically need a biopsy to confirm the diagnosis.

Keep in mind that not all lumps are cancer. If youre concerned about a lump around your abdomen, a doctor or other HCP can offer guidance and information about next steps.

    Kidney Cancer Is Often Asymptomatic

Blood in the urine can be a symptom of other ailments as well, including bladder cancer, and people who see blood in their urine should call their general practitioner to get tested. Most often when we find blood in the urine, its not cancer, its caused by other reasons. But its still important to be checked for cancer of the kidney or bladder, Dr. Kauffman says.

People who have larger tumors in their kidneys might be more likely to see blood in their urine, but it also depends on where in the kidney those tumors are growing. It doesnt necessarily have to do with how aggressive the cancer is, Dr. Kauffman says. If the tumor is near the center of the kidney where urine collects, it can lead to blood in the urine, even if the tumor is small. If a tumor is at the edge, it can grow big and not touch the area where urine collects in the center of the kidney, so there will be no blood seen in the urine.

    Renal Tumors Or Cysts

Renal tumors or cysts do not typically cause pain unless the growth is advanced or the disbursement is extensive. The three most common growth abnormalities include:

    • Renal adenoma: A type of benign tumor that can grow to a substantial size
    • Renal cell carcinoma : A type of cancer that usually begins in the tubules of the kidneys
    • Polycystic kidney disease : An inherited disorder in which benign, fluid-filled cysts proliferate throughout the kidneys

The pain would most often be unilateral and accompanied by hematuria, either visible or invisible .

If cancer is involved, persistent malaise and unexplained weight loss are telltale signs suggestive of an advanced malignancy.

PKD may also be symptom-free until such time as the formation of cysts causes structural damage to the kidney. In addition to flank pain, usually bilateral, PKD may cause progressively worsening symptoms, including headaches, high blood pressure, hematuria, abdominal pain and swelling, recurrent kidney stones, recurrent UTIs, and renal failure.

As opposed to many of the other renal conditions, PKD is associated with excessive urination rather than impaired urination. The most common form of PKD, known as autosomal dominant PKD, manifests with symptoms when patients are in their 30s and 40s. Around 10% will advance to renal failure.

When Should You Go See Your Doctor About Back Or Kidney Pain

    Back pain

Back pain can be incredibly frustrating, but the good news is: It usually gets better on its own. MCG doctors suggest avoiding aggravating movements and giving heat, ice, or anti-inflammatories a try. If it doesnt clear up within six to 12 weeks, go see your primary care doctor who can assist you in finding better treatment. If, however, you are experiencing back pain after traumatic accident or have other concerning symptoms, head over to urgent care for immediate treatment.

    Kidney pain

If you suspect that you have a kidney issue, you should always schedule an appointment with your primary care physician. Really anyone who has symptoms of a urinary tract infection or kidney stones should follow up with their physician to make that determination, says Santiago.

If you are pregnant and experiencing a significant fever, see emergency care to get evaluated right away, adds Santiago. Pregnant women are at a greater risk for complications so if primary care isnt available, urgent care is the best place to seek assistance.

    Autosomal Dominant Polycystic Kidney Disease

Where Do You Feel Kidney Pain In The Back?

This genetic disease causes numerous fluid-filled cysts to develop in the kidneys that often interfere with kidney function. The cysts also cause medical problems like infections, kidney stones, and high blood pressure if left unchecked. Some PKD patients might not experience symptoms for years until it starts causing painful symptoms.

    Diagnosis and Possible Causes

ADPKD occurs because of a problem in one of the two genes of the DNA. PKD1 and PKD2 are the genes responsible for creating proteins in the kidney cells, which is why a problem in either of these genes might cause the kidney cells to form cysts.

If the primary physician suspects that their patient has a serious kidney problem, they may refer the patient to a nephrologist. The kidney specialist then asks different questions that help them determine the patients exact condition. They may also perform tests like MRI, CT scan, and DNA test to get an accurate diagnosis.

    Care and Treatment

Unfortunately, there isnt a cure for ADPKD. Patients may only treat the health problems associated with the disease to prevent kidney failure. Depending on the complication being treated, doctors might prescribe pain medicines and antibiotics.

    Symptoms Of Kidney Cancer

Kidney cancer may not cause any signs or symptoms in its early stages. It is usually found on an ultrasound or x-ray done for another reason. Signs and symptoms often appear as the tumour grows into surrounding tissues and organs. Other health conditions can cause the same symptoms as kidney cancer.

See your doctor if you have these signs or symptoms:

    • blood in the urine
    • pain in the back and side of the abdomen
    • a lump that can be felt in the abdomen
    • swelling of the legs and ankles
    • high blood pressure
    • low red blood cell count
    • fatigue
    • for men, a large swollen vein in the scrotum

    Hidden Signs Of Kidney Cancer

  • Blood in Urine

The most common symptom and even the slightest tinge of red is a sign there is something not right with the kidney. It might be an infection, but cancer or not, blood in the urine must be immediately investigated.

  • Abdominal Lump / Mass

Another very common symptom, this can be in the abdominal region and feels like a hard, dense bulge under the skin. While it is a common symptom, it does not present itself in the early stages.

  • Lower Back Pain

While unnatural aches and pains are usually a natural part of ageing, they can at times be early warning signs of kidney cancer. Lower back or side pains can be late-stage symptoms of Kidney Cancer. What may be helpful is to know that the type of pain is not like a typical muscular pain but is more like a sharp stabbing pain which can persist as a dull ache across multiple days.

  • Loss of Appetite and Sudden Weight Loss

A sudden loss of appetite or abrupt weight loss can be a warning sign of advancing cancer. In the case of renal cancer, this symptom could indicate that cancer has spread to other parts.

  • Scrotum Swelling

Men might see the sudden appearance of swollen veins in their scrotum . This might be caused due to a kidney tumour and can affect the flow of blood to the scrotum.

  • Intermittent Fever

Fever that is not accompanied by flu symptoms but goes on for a few weeks might be a symptom and you need to get it tested.
